Reporting a problem with a record

Please include the page address and what you believe is wrong. We will check it against the official source.

• If our page does not match the official record, we will correct it.
• If the official record has been updated since our last import, we will refresh it.
• If you believe the official record itself is wrong, you need to raise it with the health department that issued it. We cannot change what the official record says.
